What Are PDO and Cog Threads?

PDO (Polydioxanone) threads are medical-grade, dissolvable sutures that have been used safely in surgery for decades. In aesthetics, they can to lift, tighten and stimulate collagen production in the skin.

There are two main types used:

  • Smooth PDO Threads: These are fine threads placed under the skin to stimulate collagen and improve skin texture, elasticity, and firmness over time. They are ideal for treating fine lines, crepey skin, and mild sagging.

  • Cog Threads (Barbed PDO Threads): These are thicker threads with tiny barbs that anchor into the tissue, providing an immediate mechanical lift. They're typically used to reposition sagging tissue in areas such as the cheeks, jawline, jowls, and neck.

 

Both types of threads gradually dissolve over six months, but the collagen stimulation continues, giving long-lasting improvements to skin tone and structure.

Your Consultation: Your experience begins with an in-depth consultation where Ophelia will assess your facial structure, skin quality and treatment expectations. She’ll recommend the most suitable approach for you and answer any questions you may have. You’ll never be rushed — this is about building trust and choosing the right plan together.

Treatment Day: The treatment area will be carefully cleansed and a local anaesthetic applied to ensure your comfort throughout. Using precise techniques, Ophelia will gently insert the threads under the skin using a fine cannula or needle, depending on the area and type of thread. Most procedures take around 30–60 minutes. You may feel some pressure or tugging, but discomfort is minimal.

Aftercare & Recovery: Following your treatment, you may notice mild swelling, tenderness or bruising, which typically resolves within a few days. You’ll receive full aftercare instructions and advice on what to avoid (such as facial massage, high-intensity exercise and dental work) during the short recovery period. Most clients return to normal activities within 24–48 hours.

Your Results: Some lifting is visible immediately — especially with Cog threads — but the real transformation happens gradually over the following 8–12 weeks. Your skin will become firmer, plumper and more youthful-looking, with results lasting up to 18 months. To help your skin heal and ensure the best possible results, it’s important to follow the recommended aftercare guidelines.

What You Can Do

  • Return to normal daily activities after 24–48 hours, depending on how you feel.

  • Use gentle skincare products (avoid active ingredients like retinol or acids for a few days).

  • Sleep on your back with your head slightly elevated for 3 to 5 nights.

  • Apply a cold compress if needed to reduce swelling or bruising.

  • Attend any recommended follow-up appointments with Ophelia.

 

What You Should Avoid

  • Do not apply makeup to the treated area for at least 24 hours.

  • Avoid strenuous exercise or high-impact activity for 7 to 10 days.

  • Do not book facials, massage, or dental work for at least 2 weeks after treatment.

  • Avoid exaggerated facial expressions and excessive chewing for the first few days.

  • Do not touch, press, or rub the treated area — let the threads settle naturally.

  • Avoid alcohol, saunas, steam rooms and very hot showers for at least 3 days.

  • Do not sleep on your side or stomach for at least a week to prevent pressure on the area.

How long does the procedure take? Typically between 30-60 minutes, depending on the number of threads and areas treated.

Is it painful? Local anaesthetic is used to numb the area, so most clients experience minimal discomfort during the procedure. You may feel slight pressure or pulling as the threads are inserted.

What’s the downtime? Some swelling, mild bruising, or tenderness may occur for a few days. Most clients resume normal activities within 24–48 hours, though strenuous exercise and facial massage should be avoided for at least a week.

How long do results last? Initial lifting effects are visible immediately, with full collagen-stimulating benefits continuing to develop over 2–3 months. Results typically last 12–18 months, depending on age, lifestyle and skin condition.

Are the threads visible or felt under the skin? No. When placed correctly, threads are not visible or palpable. Any initial tightness or sensation will subside as the threads integrate into the tissue.
